CCS faced the challenge of transforming their extensive proprietary data, accumulated over 30 years, into actionable insights that could enhance holistic health management and prevent co-morbidities in diabetes patients. They needed a solution that would not only support personalized patient outreach but also reinforce a proactive approach to chronic care management, moving beyond their traditional role as a medical equipment supplier.
To address these issues, CCS partnered with Accenture to develop the PropheSee™ platform, utilizing advanced analytics and AI. The solution integrated CCS’s internal data with social determinants of health to create a comprehensive longitudinal view of each patient via the Customer Analytics Record (CAR). Multiple predictive models were evaluated, with the selected model achieving 85% accuracy in forecasting patient behavior nearly three months in advance, and seamlessly integrating into CCS’s existing Azure Data Lake environment. PropheSee™ enabled targeted intervention programs by generating personalized messages to keep high-risk patients compliant with their treatment plans, ultimately improving treatment adherence by up to 50%.
Quantifiable benefits included a reduction in healthcare costs of up to $2,200 per patient annually, driven by increased adherence to continuous glucose monitoring (CGM), improved glycemic control, and waste elimination in CGM setup expenses. The system’s ability to proactively identify and engage high-risk individuals resulted in better clinical outcomes, notably reducing the likelihood of ER visits and hospitalizations, while supporting CCS’s strategic transition to a proactive, data-driven chronic care organization. This tailored, insights-driven approach reinforced CCS’s commitment to delivering more personalized, effective care that benefits both patients and the broader healthcare ecosystem.
